A tragic collision at the intersection of N Sheridan Rd and E 136th St N on the night of August 17, 2024, resulted in one fatality and left four individuals injured. The accident occurred at approximately 11:39 PM, about one mile south of Collinsville, in Tulsa County.

According to authorities, a 2004 Dodge Dakota, driven by a 16-year-old white male from Collinsville, was traveling westbound on E 136th St N when it failed to stop at a stop sign. The vehicle collided with a 2015 GMC Yukon traveling northbound on N Sheridan Rd.

The driver of the Dodge Dakota was pronounced dead at the scene by Collinsville Rural EMS. The identity of the juvenile driver is being withheld due to privacy concerns.

In the GMC Yukon, the driver, identified as 37-year-old Trisha Lee from Collinsville, sustained severe injuries and was transported by Owasso EMS to Saint Francis Hospital in Tulsa. She remains in critical condition with significant trunk internal and leg injuries.

Larry Lee, a 43-year-old passenger in the Yukon, was transported by Collinsville EMS to Saint John Hospital in Tulsa, also in critical condition with head and trunk internal injuries. Two juvenile passengers from the Yukon were also taken to local hospitals. An 11-year-old boy was transported to Saint Francis Hospital and is reported in fair condition with leg injuries. A 1-year-old child was taken to Saint Francis Hospital as a precaution but was not admitted.

The collision caused both the driver and a passenger in the Yukon to be pinned inside the vehicle for approximately 20 minutes. Collinsville Fire personnel used extrication tools to free them from the wreckage.

The cause of the collision is currently under investigation, including the condition of the driver of the Dodge Dakota and whether seatbelts and airbags were properly used and deployed. The weather conditions at the time of the crash were clear, and the roadway was dry.

Authorities continue to investigate the circumstances leading up to this incident.
